Start with the contact template to keep the first export easy to inspect. It asks for a name, email, optional phone, a subject choice and a message. Replace the subject choices with those relevant to the destination site. Add a clear instruction about how the completed answers will be delivered. If the form is only a prototype, say so. If you plan to adapt its collection method, complete that work and test it before inviting visitors to submit real information.
An HTML output should be tested as an output, not just as a download. Open the file in a fresh browser tab, check the title and labels, try missing required answers and enter a valid response. Confirm that the completion behavior produces the expected local answer file and that its values match what you typed. Also check radio groups and checkbox answers, because a single-choice question and a multiple-choice question should not collapse into the same kind of value.
Visual and behavior-rich features need extra attention in standalone exports. A hosted signature pad, multi-step branching, quiz scoring and cloud analytics are not automatically equivalent to ordinary HTML inputs. The exporter should make unsupported controls explicit instead of silently presenting them as working text fields. If a form depends on a feature that is not implemented in the export, keep the hosted version or simplify the exported form. User-entered text must remain text in the generated document. A title, description or field label containing angle brackets should not become executable markup. The exporter escapes those values.
